After you have applied:
1. After your application is completed, it will be reviewed and
information verified.
2. When your application is approved, you will be called and given an
appointment
time for treatment. At this time you will be informed of your co-pay.
You
will be
charged on a sliding scale based on your income.
3. You must pay for treatment when you come for each appointment. We do
not
accept checks or debit/credit cards.
NOTICE:
Be sure to call at least
24
hours in advance if you cannot keep your
appointment.
There is a $25.00 charge for all broken appointments, this must be paid
before
you can reschedule your appointment.
